| Please follow the steps below to schedule an appointment for a
sleep study with Southeastern Lung Care. |

***All patients must be referred by a physician
to have a sleep study*** |

1) |
Obtain order for sleep study. If you don’t have a normal doctor to order the
study, you may schedule an appointment with one of our doctors. |

2) |
Call the Sleep Center for an appointment either with one of our doctors, or for
the sleep study if the order has already been obtained
|

3) |
Download the sleep paperwork from our website by clicking
here. |

4) |
Arrive at the Sleep Center at your designated appointment time |
